- Bequest of Samuel D. Gross, M.D., to the Philadelphia Academy of Surgery; on permanent deposit at the College of Physicians of Philadelphia.
- With four ms. ownership inscriptions: (1) on inserted leaf before title leaf, F. B. Osiander, 1807, in red ink; (2) on the title leaf, early owner's name [C-----] mostly erased, dated 1564; (3) beneath the erasure on the title leaf, inscription of an unidentifiable Augustinian monastery, dated 1680, followed by "sum Jöachimi Middendorfij D."; and (4) on leaf a2, the autograph of D. Gesenius, whose bookplate is affixed to the front pastedown.
- Bound in half cream modern vellum over white cloth boards, with gold-stamped maroon Morocco spine label. Edges stained red. Two vellum endleaves of an early rubricated ms. religious text bound in.
- Leaf size: 301 x 203 mm. Trimmed with slight loss of marginal ms. annotations. Heavy dampstaining on outer margins, some mended; text generally not affected.
