Large initial on leaf a2r illuminated with gilt and decorated with blue, green, and red ink. Coat of arms (Party per chevron gules and azure, a chevron or.) at bottom of leaf a2r. A branch of leaves in margin of leaf e1r is supplied in green and brown ink.
- Foliated incorrectly by hand; with a few contemporary ms. annotations in margins; chapters have been numbered.
- Presented to the College of Physicians of Philadelphia by George Fales Baker, prior to 18 July 1901; with the bookplate of the J. Stockton Hough Library.
- Bound in half modern vellum over marbled paper boards, with gold-stamped maroon Morocco spine labels.
- Leaf size: 284 x 197 mm. Wide margins. Dampstaining at top margin and insect damage throughout.
