  • With contemporary ms. annotations on inserted vellum endleaf.
  • Two ownership inscriptions on third endleaf from front: "Joannis Burtti ex Libris" and "IMH" [i.e. I. Minis Hays]. Above the ownership inscriptions in another hand is a slightly illegible religious inscription in Dutch.
  • Typescript letter to Wm. B. McDaniel, signed by A.C. Klebs discussing this edition pasted on second flyleaf from front.
  • Presented to the College of Physicians of Philadelphia by I. Minis Hays, M.D., in 1909.
