  • With a letter from Henry E. Hungtington Library and Art Gallery to the College of Physicians of Philadelphia, dated October 6, 1941 (San Marino, Cal.) about the facsimile reproduction made by them to complete the wanting large initial on first leaf.
  • Acquired for the College of Physicians of Philadelphia by the Woman's Medical College of Pennsylvania from Dawson's Bookshop, Los Angeles, Calif. prior to October 15, 1941. Previously owned by Sir Thomas Phillipps, and by the Huntington Library, with their shelfmark (no. 734). Two booksellers' descriptions pasted on front pastedown.
  • Binding: contemporary full brown calf over wooden boards with two brass and leather clasps (one detached); blind tooled panels ornate in floral and geometrical designs; spine rebacked in brown calf with five raised bands; title stamped on front cover.
