Large initial on leaf [1] decorated in gilt, red, blue, green, black, and white; at bottom of leaf [1] is a crest (Gules a bend or between two vines vert) flanked by a blue leaf-like decoration on the left and a green leaf-like decoration on the right. Section marks supplied in alternating red and blue on leaves [1, 51-54, 83-86, and 124-125].
  • One leaf of ms. notes about this edition is inserted at front and bears the inscription "Stockton Hough, June 19th, 1892."
  • Presented to the College of Physicians of Philadelphia by S. Weir Mitchell, M.D., prior to 16 July 1901; with the bookplates of the J. Stockton-Hough Library and Herculis de Silva.
  • Bound in gold-stamped calf, with gold-stamped red calf spine label. Edges are speckled blue and red.
