- With the bookplate of Sir Robert D'Arcy, Hildyard, Bar.; booklabel of Charles Butler, Warren Wood, Hatfield; and original ownership inscription of Conrad of Leonberg, secretary to the abbot of the Cistercian monastery at Dijon, dated 1494.
- Purchased by the College of Physicians of Philadelphia prior to 1 June 1917.
- Bound in red calf with gold-stamped borders on covers and gold stamping on spine.
- Leaf size: 282 x 184 mm. Trimmed, with slight loss of border ornament at top. Outer margin of leaf a2 replaced, with missing text and border supplied in ink. Large holes in margins, perhaps due to the removal of ms. annotations, have been filled. Former ownership inscription(?) has been erased from verso of a1. Some tears mended with acidic tape. Leaf m7 rebacked.
