  • Acquired by the College of Physicians of Philadelphia from Davis & Orioli prior to 9 Aug. 1918; with the booklabel of George Dunn of Woolley Hall near Maidenhead.
  • Bound in half modern vellum over marbled paper boards, with lettering in brown ink on the spine and red-stained edges. Text block has fallen out of backing.
  • Leaf size: 192 x 136 mm. Trimmed with some loss of ms. annotation on title leaf.
