- Early Latin ms. table of contents on first leaf; early inscription on leaf a1 (partially illegible): "Hic liber est congregationis [sanctae?] Justine Deputatus mon[asteri]o abb[at]ie florentine [illegible] no. 20"; inscription, possibly of the Badia fiorentina, on lower margin of leaf a2: "Abb[at]ie florentine"; stamp in black on first leaf depicting an eagle with over-head crown.
- Acquired by the College of Physicians with the William F. Norris fund prior to December 5, 1922. With the bookplate of Vernon on front pastedown.
- Bound in brown calf over wooden boards; blind stamped panels with floral and geometrical designs; remains of two brass clasps; brass corner pieces; five raised bands on spine; leather label on panel wit author and title on spine. Vellum endleaves with early ms. and music in red and black ink. "Averois" written in ms. on fore-edge.
