- Acquired by the College of Physicians of Philadelphia from Davis & Orioli, prior to 2 Dec. 1921.
- Bound in full blind-tooled brown calf, with gold-stamped edges and spine; textblock edges stained red. Front board detached; housed in brown buckram clamshell box.
- Leaf size: 146 x 100 mm.
