- With the booklabel of Pierre Duhem affixed to the verso of the front flyleaf.
- Acquired by the College of Physicians of Philadelphia from Davis & Orioli, prior to 6 Apr. 1920.
- Tightly bound in full modern vellum with gold-stamped red Morocco spine label.
- Leaf size: 291 x 202 mm. Closely trimmed. Wormholes throughout. Many wormholes and corners have been mended with acidic tape. The upper and lower margins of the first four gatherings and a few holes elsewhere have been mended with paper.
