  • Presented to the College of Physicians of Philadelphia by Charles W. Burr, M.D., and Edward B. Krumbhaar, M.D., prior to 11 Dec. 1928; with the booklabel of Charles Fairfax Murray on the front pastedown. An initialed typescript note from Arnold C. Klebs, dated 2 Apr. 1931, pasted on an inserted leaf at front, discusses the typography and illustration in this edition.
  • Bound in gold-stamped full rust Morocco by Katherine Adams, 1902; signed binding.
  • Leaf size: 203 x 150 mm. Dampstaining in upper and lower margins; wormholes in the lower margin of last gathering have been mended, without replacing slight loss of text.
