- Foliated incorrectly in ms.; with contemporary ms. annotations and fists in margins.
- Presented to the College of Physicians of Philadelphia by John K. Mitchell, M.D., prior to 17 July 1901; with the bookplate and bookstamp of the J. Stockton-Hough Library and the bookplate of John Hope, the 4th Earl of Hopetoun (Arms: Azure on a chevron or between three bezants a bay leaf, slipped vert. Crest: A globe, fracted at the top, under a rainbow with clouds at each end, all proper.)
- Bound in gold-tooled speckled brown calf, with gold-stamped maroon Morocco spine label. Spine is broken and taped together with brown cloth tape; housed in brown buckram clamshell.
- Leaf size: 196 x 137 mm. Trimmed with slight loss of ms. annotations in margins.
