  • Presented to the College of Physicians of Philadelphia by Charles W. Burr, M.D., prior to 9 April 1935; received from Davis & Orioli on 4 April 1935. With the bookplate of Sidney Graves Hamilton.
  • Bound in stamped brown calf over oak boards; with gold-lettering on the spine.
  • Leaf size: 294 x 199 mm. Wide margins. The margins of the penultimate leaf have been mended with paper. Slight dampstaining in lower margins. Worm damage to margin of last gathering.
