Three contemporary ownership inscriptions are written on the recto of the first leaf (blank): "Ex libris Fr[atr]is Caroli Manzini Carmel. a Regio.", "Herculis Fabalij", and "Aloisius [illegible]".
- Presented to the College of Physicians of Philadelphia by Charles W. Burr, M.D., prior to 9 April 1935; received from Davis & Orioli on 4 April 1935. With the bookplate of Sidney Graves Hamilton.
- Bound in stamped brown calf over oak boards; with gold-lettering on the spine.
- Leaf size: 294 x 199 mm. Wide margins. The margins of the penultimate leaf have been mended with paper. Slight dampstaining in lower margins. Worm damage to margin of last gathering.
