  • Imperfect: Leaf a1 (blank) wanting; first quire misbound as follows: leaf a7 bound between leaves a2 and a3; two photostats of a7 recto and verso misbound in reverse order between leaves a6 and a8. Leaf a2 has been mended at top, with text supplied in ms.: "sanitatis" in caption title spelled "sanitalis".
  • Rubrication: Capital strokes, paragraph marks, and underlinings in first quire supplied in red.
  • Foliated in ms., with ms. annotations in a contemporary hand throughout.
  • Purchased by the College of Physicians of Philadelphia prior to 17 Feb. 1917.
  • Bound in brown paper boards. Spine loose; housed in gold-stamped brown buckram clamshell. First quire has been reinforced with paper guards.
  • Leaf size: 200 x 137 mm.
