- Some early Latin ms. notes in margins; foliated in ms.
- With an early ownership inscription of the Benedictine Kloster Altomünster (Germany) dated 1543 in red and black ink on front pastedown ("Monasteriu[m] Althominsteri 15:43, Scala Celi") and the Monastery's ms. title and shelfmark label in red and black pasted to front cover.
- Acquired by the College of Physicians of Philadelphia from Taeuber & Weil prior to March 14, 1930. Bookseller's description pasted to recto of first leaf.
- Bound in contemporary blind tooled brown calf over wood boards; remains of two clasps; three raised bands on spine; ms. label with title on front cover; title written in ms. on upper and lower edges.
